Please complete the New Practice Member forms before visiting the practice. This is where you document your health concerns and health history. Completing online saves time; otherwise, please arrive 15-20 minutes before your first appointment to complete. Please bring your ID and insurance information as well.
New Practice Member Forms
The Initial Consultation
Before you meet the doctor, we’ll give you a quick tour of the office. During the initial health consultation, you can share what’s preventing you from fully enjoying health, wellness, and vitality. The doctor will assess your current well-being, and find out what your health goals are.
Next, we’ll perform a comprehensive wellness examination, testing your reflexes, range of motion, and other neurological, orthopedic, postural, and physical examinations. We use state-of-the-art scanning technology to ensure accurate measurements, reporting, and tracking. Digital X-rays are taken at this time, if needed.
When You Return
Your Report of Findings is a detailed analysis of how to solve your problem and prevent a recurrence. We’ll look at your current lifestyle choices, goals for the future, and any challenges standing in your way. Our recommendations are extremely comprehensive to help you find the right solution.
Ongoing Chiropractic Care
We consistently update and customize your wellness programs. The doctor gives adjustments during your visits; CA’s administer adjunctive therapies to support the adjustment based on the doctor recommendations. Stretching, exercise instruction, nutrition counseling, or other advice, may be part of the doctor’s plan for you. Questions are always welcomed and encouraged.
Finances
Our staff will do a complimentary insurance verification to find out how much your insurance covers. All financial options are discussed with you, so there are never any surprises.
We’re in-network with most insurance carriers, and accept Medicare, Medicaid, HSA plans, and credit cards. Please speak with our staff for further information.
